Broken links across the site
A broken link in a menu, footer or template appears on many pages at once, so one fix repairs them all.

How Lastmod finds it
The crawl records which link targets return an error and groups the ones that appear on a large part of the pages.
What you do
Fix each link once where it comes from, or restore the target page.
How the work is followed up
Lastmod looks for your change on the live page and closes the opportunity when the problem is gone. Clicks are followed as context, not as the proof.
